In 2000, Dr. Delmont founded Delmont Medical Care, a multi-site, multi-specialty, patient-centered medical home level 3 practice with 20 primary care providers, delivering culturally sensitive, ambulatory, efficient and high-quality medical care to 25,000 underserved populations across Long Island and Queens, insured predominantly by Medicaid (60%).
Dr. Delmont’s endeavors have rendered her an expert papertyper.net on various subjects. As an early adopter of a patient-centered team-based approach and Electronic Medical Records, she quickly became a pioneer in the field of medical practice transformation and Patient Centered Medical Home.
As an advocate, she collaborates with various not-for-profit organizations, to conduct community outreach to create awareness on preventative health, chronic diseases as well as self-management.
Between 2012 and 2015 Dr. Delmont was the Chief Medical Officer at Beacon Health Partners, a Long Island based Independent Practice Association and Accountable Care Organization with over 1000 physicians participating in the Medicare Shared Savings program and commercial payer contracts. As Chief Medical Officer, Dr Delmont was responsible for care coordination, disease management, quality assurance and compliance requirements. In addition, she established and implemented clinical guidelines and patient-centered medical home transformation.
In July 2019 Dr. Delmont joined SOMOS IPA as the Chief Medical Officer. Together with the leadership team, she is tasked to develop a modular population health program that can be customized for both full risk and partial risk contracts across the spectrum of IPAs and lines of business. Dr. Delmont’s experiences have given her insight into quality healthcare delivery. Dr. Delmont’s contribution will be key in bridging the gap between clinical and business elements within the value-based care model.
