Form I-693 Report of Medical Examination and Vaccination Record

Immigration exams are required for all persons applying for adjustment of status by the Department of Homeland Security. Dr. Jacqueline Delmont is a United States Citizenship and Immigration Services (USCIS) designated civil surgeon licensed by the government to perform these exams.

Medical Requirements:

A physical exam by a USCIS designated civil surgeon including vital signs, tuberculosis testing (QuantiFERON), Syphilis testing and age appropriate vaccination administration. Chest x-rays are required only if you are QuantiFERON positive. We will provide you with a copy of the I-693, Report of Medical Examination and Vaccination Record in a sealed envelope and a copy for your records. You do not have to bring your own.

Form N-648 Medical Certification for Disability Exceptions

Form N-648 is for applicants who seek an exception to the English and/or civics requirements of the naturalization process due to a physical or developmental disability or mental impairment that has lasted or is expected to last twelve months or more.

A Form N-648 applicant should submit Form N-648 at the time he or she files Form I-400, Application for Naturalization. Form N-648 applicants are not required to fulfill the English language and civics requirements if the applicant is unable to do so due to physical or mental disabilities. Illiteracy alone is not a valid reason to seek an exception under Form N-648.

The medical professional must explain in plain English and layman’s terms how the provider diagnosed the impairment and how the diagnosed condition affects the applicant’s ability to learn or demonstrate English proficiency and/or knowledge of U.S. history and government.

Form N-648 must be certified by a licensed medical professional. Before certifying Form N-648, the medical professional must have conducted an examination of the applicant. Form N-648 responses must be indicated in common language, without abbreviations, so that a person without medical training would be able to understand. Only medical doctors, doctor of osteopathy, or clinical psychologists licensed to practice in the United States may complete Form N-648.

Medical Requirements:

A physical examination, review of previous medical records, addtional pertinent diagnostic tests to prove where is the limitation that does not allow the applicant to learn and answer the questions asked by the immigration office during the naturalization interview.
